Adult day health care is only one of the care services offered through the PACE program. Bluegrass PACE Care is a Medicare and Medicaid program that provides comprehensive health care services and helps a person live in the community safely. The PACE Center has activity rooms, a computer center, an outdoor patio, commercial kitchen, therapy gym, wheelchair accessible shower rooms, laundry facility, beauty shop, and a medical clinic.
Eligibility for enrollment in the PACE program:
* 55 years old or older
* Live in the service area of a PACE organization – Bluegrass Care Navigators service area includes Woodford, Anderson, Fayette, Jessamine, and Franklin counties in Kentucky
* Certified by the state in which you live as meeting the need for nursing home level of care – minimum of 2 indicators
* Able to live safely in the community with the help of PACE services at the time of enrollment.
PACE (Program for All-Inclusive Care of the Elderly) provides all needed medical and supportive services for eligible older adults, including:
* Preventative care
* Support with chronic disease symptoms
* Adult day primary care including physician and recreational services
* Daily activities of living including bathing, dressing and eating
* Medical specialty care
* Nutritional support
* Occupational and physical therapy
* Dentistry
* Hospital and emergency care support and management
* Prescription drugs and medication management
* Counseling and social work support
* Social services including caregiver support and respite care
* Transportation to the PACE center for activities and appointments
