You’ve discussed, argued, and pleaded, but your mother will not move out of her home despite her lung condition that requires therapy twice a week plus frequent doctor visits. She can’t drive and is lonely. Meanwhile, you’re overwhelmed with work and family responsibilities. You worry about her and feel guilty your visits are all about tasks instead of sharing quality time.
This is a familiar story for aging people with health conditions living at home and their caregivers. But now there’s an answer: the new Care Guide Partners Program of All-Inclusive Care for the Elderly (PACE).
PACE is a certified program funded by Medicaid and Medicare. Founded in the 1970s, it helps seniors stay in their homes and communities instead of relocating to nursing facilities. Now, over 72,000 people participate in 160 programs in 32 states.
From its new PACE Center in Elizabethtown, Care Guide Partners’ team of geriatric specialists coordinate and deliver all the medical care, senior services, transportation, and in-home support participants need to remain independent. It’s a “one-stop shop” of support for seniors wanting to age safely and comfortably at home. It’s also a big help to their loved ones.
PACE offers:
- 24/7 comprehensive health and medical care, including primary care and specialist services
- Outpatient labs, X-rays, dental, podiatry, optometry, audiology care, and medical supplies
- Rehabilitation services, meals, and nutrition planning
- Adult day programming with social and recreational activities
- In-home equipment and support with activities of daily living, including bathing, dressing and eating
- Prescription drugs and medication management
- Transportation to and from the PACE Center and all program-coordinated appointments
To be eligible for PACE, seniors must:
- Be at least 55 years old
- Be certified by the Commonwealth of Kentucky to need nursing home-level care
- Be able to live safely in the community at the time of PACE enrollment
- Be living in Bullitt, Grayson, Hardin, Hart, LaRue, or Nelson counties
